Pre-registration forms must be received by August 15, 2008 to avoid a late registration fee of $10.00. Make checks payable to Dallas Area Council of the Blind (DACB). There are two forms available for pre-registration. There is a manual form that you can simply print, fill it out, and mail with your check to cover your selected expenses.
There is also a pre-registration form in word format for better compatibiltiy with screen readers.
There is also an automatic form that you can fill out in the computer. Before you enter the documnet, be sure to read my accessibility guide. Once you have completed filling out the form, you should save your form, print it, sign it and mail it with your check to cover your selected expenses. Each family member attending the conference must fill out a form and submit it with a check to cover selected expenses. Save each form you filled out to your computer using a different name so that you will have a record of all information submitted to the Dallas Area Council of the Blind. Unless you manually email the form to a friend or relative, our forms will not automatically send any data to American Council of the Blind, Dallas Area Council of the Blind, American Council of the Blind - Texas Inc., or to anyone else.
